Purpose of the Study:

To explore diagnostic challenges in acute cholangitis.
To highlight the importance of comprehensive assessment for obstructive jaundice.
To raise awareness of hematologic malignancies mimicking liver disease.

Main Methods:

Case report of a 38-year-old woman with Crohn's colitis.
Initial presentation with abdominal pain, jaundice, and leukocytosis.
Unexpected peripheral smear findings leading to revised diagnosis.

Main Results:

Initial suspicion of acute cholangitis was replaced by B-lymphoblastic leukemia diagnosis.
Leukemic infiltration of the liver caused obstructive jaundice.
