Impotence and bicycling. A seldom-reported connection.

S Solomon, K G Cappa

    Postgraduate Medicine
    |January 1, 1987

    Summary

Bicycling can cause temporary penile numbness and long-term erectile dysfunction due to nerve and blood vessel compression. Resolving these issues involved adjusting the bicycle seat and stopping exercise, restoring sexual function.

    Area of Science:

    • Urology
    • Sports Medicine
    • Neurology

    Background:

    • Erectile dysfunction (ED) is a common condition with multifactorial causes.
    • Bicycle riding is a popular exercise with potential health benefits.
    • Previous research has suggested potential links between cycling and sexual dysfunction.

    Observation:

    • A case study involving a male cyclist experiencing penile numbness and progressive erectile dysfunction.
    • Symptoms included transient penile sensations during exercise and impaired sexual potency over a year.
    • Resolution of symptoms occurred after lowering the bicycle seat and discontinuing exercise.

    Findings:

    • The patient's symptoms resolved upon modifying the bicycle setup and ceasing the activity.
    • This suggests an ischemic (blood flow restriction) mechanism for penile sensation abnormalities.
    • A neural (nerve-related) mechanism was implicated in the development of impotence.

    Implications:

    • Bicycle riding may contribute to sexual dysfunction through vascular and neural compression in the perineal region.
    • Healthcare providers should consider cycling as a potential factor in patients presenting with sexual dysfunction.
    • Further research is warranted to explore the prevalence and mechanisms of cycling-related sexual dysfunction.
