    Background:

    • Insomnia is characterized by subjective complaints of poor sleep quality.
    • Objective measures of sleep often do not fully align with subjective reports in individuals with insomnia.

    Purpose of the Study:

    • To compare objective sleep parameters and subjective sleep experiences between insomniacs and healthy controls.
    • To investigate arousal thresholds and their relationship to perceived sleep quality in insomnia.

    Main Methods:

    • Polysomnography was used to measure baseline sleep in ten insomniacs and matched controls.
    • Arousal thresholds were assessed using auditory stimuli (tones and a recorded voice).
    • Retrospective sleep reports and habitual sleep questionnaires were administered.

    Main Results:

    • Insomniacs and controls showed minimal differences in objective sleep measures and auditory arousal thresholds.
    • Insomniacs significantly overestimated awakenings and sleep duration compared to controls.
    • Subjective sleep quality correlated with REM sleep and negatively with slow-wave sleep in both groups.

    • Subjective insomnia is not solely explained by "lighter" sleep or increased objective sleep disruption.
    • Perceptual distortions of sleep, particularly overestimation of awakenings, play a crucial role in the experience of insomnia.
    • Understanding the discrepancy between subjective and objective sleep is key for effective insomnia treatment.
