    Background:

    • Sinus histiocytosis with massive lymphadenopathy (SHML) is a rare benign proliferation of histiocytes.
    • Renal involvement by SHML is exceptionally uncommon.
    • Distinguishing benign conditions from malignant neoplasms in the kidney is crucial for appropriate patient management.

    Observation:

    • A 26-year-old woman presented with an infiltrative renal lesion and significant regional lymphadenopathy.
    • Histologic examination of both the renal mass and a concurrent orbital mass revealed identical features of sinus histiocytosis.
    • Radiographic imaging suggested an infiltrative renal neoplasm.

    Findings:

    • The renal mass and orbital mass demonstrated histopathologic concordance for sinus histiocytosis.
    • The infiltrative nature of the renal lesion mimicked aggressive renal neoplasms.
    • This case highlights the potential for benign sinus histiocytosis to present as a renal mass.

    Implications:

    • Sinus histiocytosis should be considered in the differential diagnosis of infiltrative renal lesions, particularly in younger patients.
    • Radiographic and histologic correlation is essential to avoid misdiagnosis of renal cell carcinoma, lymphoma, or leukemia.
    • This case underscores the importance of comprehensive evaluation for unusual presentations of benign conditions.
