Fixation and Sectioning

Two basic types of preparation are used to visualize specimens with a light microscope: wet mounts and fixed specimens.
The simplest type of preparation is the wet mount, in which the specimen is placed in a drop of liquid on the slide. A liquid specimen can be directly deposited on the slide using a dropper. Background:

Diabetic foot ulcers are prone to severe infections, often involving multidrug-resistant organisms.
Chronic osteomyelitis requires aggressive treatment, including surgical debridement and targeted antibiotics.
Cefiderocol is a novel siderophore cephalosporin effective against certain Gram-negative pathogens.

Observation:

A patient with extensively drug-resistant (XDR) Pseudomonas aeruginosa, XDR Acinetobacter baumannii, and Enterococcus faecalis in a diabetic foot ulcer was treated with ampicillin-sulbactam and cefiderocol.
The patient developed brown urine discoloration during cefiderocol treatment.
Investigations ruled out hemoglobinuria, myoglobinuria, and bilirubinuria as causes for the urine discoloration.

Findings:

Brown urine discoloration was suspected as a side effect of cefiderocol.
Discontinuation of cefiderocol led to the resolution of urine discoloration within 3 days.
This suggests a potential, reversible adverse effect of cefiderocol therapy.

Implications:

Clinicians should be aware of potential cefiderocol-associated urine discoloration.
Monitoring for adverse effects is crucial when using novel antibiotics like cefiderocol.
Further studies are needed to elucidate the mechanism and incidence of this cefiderocol side effect.
