Main Results:

  • 1,079 patients included (mean age 41.8 years, 44.6% male).
  • Degenerative disk disease was the most common indication (81.8%).
  • Pooled fusion rate: 78.9% (95% CI, 68.9-90.4).
  • Complications occurred in 19.2% (95% CI, 13.4-27.4).
  • Conversion to open surgery required in 7.2% (95% CI, 4.6-11.4).

Conclusions:

  • Current literature does not strongly support L-ALIF.
  • High complication and conversion rates warrant consideration.
  • The limitations of endoscopic approaches in ALIF do not preclude their use in posterior spinal procedures.
