Summary

Primary trigeminal neuralgia (PTN) involves widespread white matter abnormalities, particularly reduced fractional anisotropy (FA). These microstructural changes in the brain may contribute to the development and progression of this chronic pain condition.

Background:

  • Primary trigeminal neuralgia (PTN) is a common chronic pain condition.
  • Its underlying mechanisms extend beyond the trigeminal system.
  • Comprehensive data on whole-brain white matter involvement in PTN is limited.

Purpose of the Study:

  • To investigate white matter microstructural integrity in patients with PTN using diffusion tensor imaging (DTI).
  • To explore the relationship between white matter abnormalities and clinical characteristics in PTN.

Main Methods:

  • A prospective case-control study involving 60 PTN patients and 28 healthy controls.
  • Diffusion tensor imaging (DTI) with tract-based spatial statistics (TBSS) was employed.
  • Analysis included fractional anisotropy (FA), mean diffusivity (MD), radial diffusivity (RD), and axial diffusivity (AD), with ROI-based comparisons and correlation analyses.

Main Results:

  • A widespread, symmetrical reduction in FA was observed in PTN patients compared to controls (p < 0.05).
  • Significant FA decreases were noted in the corona radiata, internal capsule, optic radiation, thalami, and cerebellar peduncles.
  • No significant differences in MD, AD, or RD maps were found; trends towards decreased FA were observed in the internal capsule and inferior fronto-occipital fasciculus.

Conclusions:

  • Individuals with primary trigeminal neuralgia exhibit white matter microstructural abnormalities.
  • These abnormalities, particularly reduced FA, may play a role in the pathogenesis and progression of PTN.
