    • A review of 135 hepatectomy cases was conducted.
    • Eight patients (5.9%) died postoperatively.
    • Seven of the deaths were attributed to liver failure coma.

    Findings:

    • Preoperative misjudgment of liver functional reserve is a significant issue.
    • A two-session hepatectomy approach following neoadjuvant chemotherapy is explored.
    • This strategy is considered for large liver tumors in patients with reduced liver function.

    Implications:

    • Improved patient selection and risk stratification are needed for hepatectomy.
    • Staged surgical interventions may offer a safer alternative for complex cases.
    • Further research into optimizing liver function assessment is warranted.
