Low blood levels of the thyroid hormones — triiodothyronine (T3) and thyroxine (T4) — signal the hypothalamus to release the thyrotropin-releasing hormone (TRH). TRH then reaches the pituitary gland and stimulates the release of thyroid-stimulating hormone(TSH) into the bloodstream.

    Summary

    Hashimoto's thyroiditis significantly increases thyroidectomy operation and recurrent laryngeal nerve (RLN) identification times. This study confirms Hashimoto's thyroiditis as a factor contributing to difficult thyroidectomies.

    Background:

    • Hashimoto's thyroiditis (HT) is often considered a challenging condition for thyroidectomy.
    • Limited data exists to definitively classify HT as a cause of difficult thyroidectomy.

    Purpose of the Study:

    • To investigate whether Hashimoto's thyroiditis is associated with increased surgical difficulty during total thyroidectomy.
    • To compare operative times and recurrent laryngeal nerve (RLN) outcomes in patients with and without HT.

    Main Methods:

    • A retrospective analysis of 614 patients undergoing total thyroidectomy.
    • Exclusion of 270 patients with pre-existing factors prolonging surgery.
    • Comparison of two groups: 132 patients with HT and 212 patients with other benign thyroid diseases.

    Main Results:

    • Patients with HT showed significantly longer operation times and time to identify the RLN (p<0.001 for both).
    • Statistically significant differences were also observed in patient age (p=0.01) and sex (p=0.007).
    • No significant differences were found in RLN identification rates, injury rates (transient or permanent), or other postoperative complications between groups (p>0.05).

    Conclusions:

    • Hashimoto's thyroiditis is associated with significantly longer operative times and increased difficulty in identifying the recurrent laryngeal nerve.
    • The findings scientifically support classifying HT as a condition leading to difficult thyroidectomies.
    • This classification may offer legal support for surgeons managing complex cases.
