Summary

Exercise, particularly resistance and stretching, is the most effective strategy for preventing work absence and back pain episodes. Combining exercise with education also significantly reduces pain and absenteeism days.

  • Work absence due to back pain (BP) poses a significant public health and economic burden.
  • Identifying effective prevention strategies is crucial for reducing BP-related absenteeism and episodes.

Purpose of the Study:

  • To systematically review and synthesize evidence on the most effective strategies for preventing work absence and back pain episodes.
  • To compare the efficacy of various prevention strategies using a network meta-analysis.

Main Methods:

  • A comprehensive search of randomized controlled trials (RCTs) was conducted across multiple databases (PubMed, CENTRAL, Embase) and previous meta-analyses.
  • A network meta-analysis was performed on data from 13 RCTs involving 2033 participants.

Main Results:

  • Exercise, especially resistance exercise, demonstrated the highest effectiveness in reducing the number of individuals reporting absenteeism due to BP (RR = 0.10).
  • Resistance and stretching exercises combined with education were most effective in reducing pain (RR = 0.80) and absenteeism days (SMD = -0.39).
  • Strategies like exercise with ergonomics, education alone, and back belts showed no significant effect on preventing absenteeism or BP episodes.

Conclusions:

  • Exercise, particularly resistance and stretching modalities, is the leading intervention for preventing sickness absenteeism and back pain episodes.
  • Combining exercise with educational components further enhances its effectiveness in managing pain and reducing work absence duration.
