Background:

  • Understanding predisposing factors and effective treatment strategies is crucial for managing MG.
  • Previous studies have highlighted various aspects of MG, but long-term outcomes require further investigation.

Purpose of the Study:

  • To identify contributing factors for successful long-term treatment outcomes in a large cohort of patients with malignant glaucoma.
  • To evaluate the efficacy of different treatment modalities for malignant glaucoma.

Main Methods:

  • Retrospective, interventional, consecutive case series.
  • Data from 74 eyes of 73 patients diagnosed with MG between 1997 and 2022 at the Stein Eye Institute, UCLA, were analyzed.
  • Collected data included demographics, clinical characteristics, and treatment outcomes, with anatomical success defined as normal anterior chamber depth and complete success as anatomical success plus IOP < 21 mmHg without further surgery.

Main Results:

  • Female gender, primary angle closure glaucoma (PACG), and prior glaucoma surgery were identified as predisposing factors for MG.
  • Medical treatment alone resolved MG in only 2 of 56 eyes.
  • Surgical treatment, particularly combined pars plana antero-central vitrectomy, hyaloido-zonulectomy, and iridectomy, achieved a 96.4% anatomical success rate in 55 eyes.

Conclusions:

  • Female gender, PACG, and glaucoma surgery are significant predisposing factors for malignant glaucoma.
  • Medical management alone is generally inadequate for treating malignant glaucoma.
  • A specific surgical technique involving combined pars plana antero-central vitrectomy, hyaloido-zonulectomy, and iridectomy demonstrates consistent and high long-term success rates for malignant glaucoma.
