Cardiac events (CEs) associated with human epidermal growth factor receptor 2 (HER2)-targeted antibody-drug conjugates (ADCs) are infrequent. Enhanced surveillance is recommended for T-DXd treatment and combination therapies due to higher observed incidences of CEs.

Area of Science:

  • Oncology
  • Cardiology
  • Pharmacology

Background:

  • Human epidermal growth factor receptor 2 (HER2)-targeted antibody-drug conjugates (ADCs) represent a significant advancement in treating breast cancer and other solid tumors.
  • Systematic reporting of cardiac events (CEs) associated with HER2-targeted ADCs is lacking, necessitating comprehensive analysis.
  • This study addresses the need for a systematic evaluation of CEs in patients receiving HER2-targeted ADCs.

Purpose of the Study:

  • To systematically assess the incidence of all-grade and severe (≥3) cardiac events (CEs) in patients treated with HER2-targeted antibody-drug conjugates (ADCs).
  • To identify specific HER2-targeted ADCs, trial phases, or treatment regimens associated with a higher risk of CEs.
  • To provide evidence-based recommendations for monitoring and managing CEs in the context of HER2-targeted ADC therapy.

Main Methods:

  • A systematic literature search was conducted across PubMed, Embase, Cochrane Library, and ClinicalTrials.gov up to May 2023.
  • Prospective clinical trials of marketed HER2-targeted ADCs were included, with data extracted by two independent investigators.
  • Meta-analysis using Stata 15.0 was performed to estimate pooled incidence of CEs with 95% confidence intervals, adhering to PRISMA guidelines.

Main Results:

  • Analysis of 47 trials involving 10,594 patients revealed a pooled incidence of all-grade CEs at 4.7% and ≥3 grade CEs at 0.6%.
  • The incidence of CEs leading to dosage discontinuation was 0.8%.
  • Higher incidences of all-grade CEs were observed with T-DXd compared to T-DM1 (7.7% vs. 3.6%), in Phase I/II vs. Phase III trials (6.9% vs. 3.2%), and in combination therapy vs. monotherapy (7.6% vs. 3.9%). Electrocardiogram QT corrected interval prolongation was the most frequent CE (5.9%).

Conclusions:

  • Cardiac events associated with HER2-targeted ADCs occur at a relatively low incidence.
  • Specific treatments like T-DXd and combination therapies warrant increased vigilance for CEs.
  • Enhanced surveillance measures are crucial for optimizing patient safety during HER2-targeted ADC treatment.

Heart Failure Drugs: Inhibitors of Renin-Angiotensin System01:26

Heart Failure Drugs: Inhibitors of Renin-Angiotensin System

The activation of the sympathetic nervous system and the renin-angiotensin-aldosterone system (RAAS) contributes to cardiac remodeling, and inhibiting the RAAS is a pharmacological target in heart failure management. As a result, neurohumoral modulation is a crucial treatment principle for managing heart failure. This approach involves using medications like ACE inhibitors (ACEIs), angiotensin receptor blockers (ARBs), β-blockers, mineralocorticoid receptor antagonists (MRAs), and neutral...

Cardiovascular Drugs: Classification based on Therapeutic Indications

Cardiovascular diseases, encompassing a range of conditions, can significantly affect the heart's operations and the overall circulatory system. These conditions impair the heart's ability to pump blood, leading to a deficit in oxygen supply to crucial organs. Anomalies in the heart's electrical system, known as arrhythmias, can cause heartbeats to accelerate or slow down. Usually, heart rates increase during physical activity and decrease while resting or sleeping. However,...

Targeted Cancer Therapies02:57

Targeted Cancer Therapies

The targeted cancer therapies, also known as “molecular targeted therapies,” take advantage of the molecular and genetic differences between the cancer cells and the normal cells. It needs a thorough understanding of the cancer cells to develop drugs that can target specific molecular aspects that drive the growth, progression, and spread of cancer cells without affecting the growth and survival of other normal cells in the body.
There are several types of targeted therapies against...

Heart Failure Drugs: β-Blockers01:22

Heart Failure Drugs: β-Blockers

β-adrenergic antagonists, commonly known as β-blockers, block the effects of sympathetic neurotransmitters such as noradrenaline (NA) and adrenaline (ADR). They have several beneficial effects in heart failure treatment. They reduce heart rate, the force of contraction, and cardiac muscle relaxation. They also slow the atrial-ventricular conduction rate and raise the threshold for arrhythmias. The concentration of β-blockers determines their effects on bronchodilation,...

Heart Failure Drugs: Inotropic Agents

Positive inotropic agents are commonly used as the first line of treatment for heart failure. One such agent is digoxin, derived from the genus Digitalis, which has been known for centuries but effectively utilized since 1785. However, these cardiac glycosides can have potentially toxic effects due to their mechanism of action, which involves inhibiting Na+/K+-ATPase and increasing contractility. Digoxin is absorbed orally and distributed in various tissues, including the CNS. It has a long...

Antiplatelet Drugs: Prostaglandin Synthesis, P2Y12 and Glycoprotein IIb/IIIa Inhibitors

Antiplatelet drugs emerge as frontline defenders against the insidious threat of thromboembolic diseases, where abnormal clots obstruct vital blood vessels. These drugs stand as bulwarks, inhibiting platelet aggregation and clot formation, thereby mitigating the risk of life-threatening conditions like myocardial infarction, coronary artery disease, and thrombotic strokes.
Prostaglandin synthesis inhibitors, exemplified by the widely known aspirin, wield their power by irreversibly acetylating...
