Area of Science:

  • Otolaryngology
  • Neurosurgery
  • Medical Imaging

Background:

  • Accurate vestibular implant electrode placement is critical for selective nerve stimulation.
  • Current challenges include visualizing the target during surgery, impacting electrode positioning.
  • This study addresses the need for improved surgical accuracy in vestibular implantation.

Purpose of the Study:

  • To investigate the accuracy of a novel surgical protocol for vestibular implant electrode placement.
  • To evaluate the efficacy of real-time fluoroscopy and intraoperative CT imaging in guiding electrode positioning.
  • To compare the accuracy of the new protocol against historical controls.

Main Methods:

  • A single-center, case-controlled cohort study utilizing a historic control group.
  • Patients received vestibulocochlear implants with intraoperative fluoroscopy and cone beam CT imaging.
  • Electrode position accuracy was defined as within 1.5 mm of the ampulla center and analyzed postoperatively.

Main Results:

  • The new imaging protocol enabled accurate electrode placement in all cases (21/21 electrodes).
  • The study group with intraoperative imaging showed significantly higher accuracy compared to the control group (10/15 electrodes).
  • The difference in accuracy between groups was statistically significant (p=0.008).

Conclusions:

  • Combined intraoperative fluoroscopy and CT imaging enhance electrode positioning accuracy in vestibular implantation.
  • Improved accuracy may lead to superior vestibular implant performance.
  • This imaging approach offers a promising solution for precise electrode placement in complex surgeries.
