  • Beta human chorionic gonadotropin (β-hCG) is a hormone typically associated with pregnancy, but can be aberrantly produced by certain tumors.

Observation:

  • A postmenopausal patient presented with vaginal bleeding and an elevated serum β-hCG.
  • Pelvic MRI revealed a complex uterine mass with central necrosis.
  • Surgical pathology confirmed a malignant PEComa.

Findings:

  • The patient's β-hCG levels normalized post-surgery, suggesting β-hCG secretion by the PEComa.
  • This represents a rare instance of a β-hCG secreting PEComa, with only one prior report.
  • β-hCG secreting sarcomas, including PEComas, may be associated with a poorer prognosis.

Implications:

  • The clinical significance of β-hCG in non-trophoblastic tumors like PEComas requires further investigation.
  • Understanding the role of β-hCG as a potential biomarker for treatment response and disease activity is crucial.
  • Additional research is needed to establish β-hCG as a predictive and prognostic biomarker in these rare malignancies.
