Purpose of the Study:

  • To evaluate the performance of a novel panel of methylated DNA markers (MDM) combined with hemoglobin for CRC and advanced precancerous lesion (APL) detection.
  • To assess the sensitivity and specificity of this next-generation mt-sDNA test in a simulated screening population.

Main Methods:

  • A new panel of highly discriminant candidate methylated DNA markers (MDM) was developed.
  • Performance was evaluated using archived and prospectively collected stool samples from multi-center case-control studies.
  • Sample processing involved targeted MDM capture, bisulfite conversion, MDM quantitation, and fecal hemoglobin ELISA.
  • Data were analyzed using 75%/25% training-testing sets with 1,000-fold cross-validation.

  • The next-generation mt-sDNA test achieved a mean sensitivity of 95.2% for CRC and 57.2% for APLs in verification studies.

  • APL sensitivity varied by lesion type, with higher rates for high-grade dysplasia (82.9%) and lower rates for sessile serrated lesions (30.2%).

  • The novel MDM panel combined with hemoglobin demonstrates high sensitivity and specificity for CRC detection.
  • The mt-sDNA test shows promise as a next-generation noninvasive screening tool for colorectal cancer and advanced precancerous lesions.
  • Further validation in a prospective clinical trial is warranted to confirm these findings.
