    Paraspinal lipomatosis, a benign fat deposition, can cause spinal masses. This condition may occur even without steroid use, aiding in differential diagnosis of spinal findings.

    Background:

    • Paraspinal lipomatosis is typically associated with exogenous or endogenous steroid excess.
    • It presents as a paraspinal mass, often requiring further investigation.

    Observation:

    • Two patients presented with paraspinal masses detected on conventional radiographs.
    • Neither patient had a history of steroid use or excess.

    Findings:

    • Computed tomography confirmed the paraspinal masses as benign fat deposition (lipomatosis).
    • This diagnosis obviated the need for more invasive diagnostic procedures.

    Implications:

    • Paraspinal lipomatosis can manifest as a benign paraspinal mass.
    • It should be considered in the differential diagnosis of paraspinal masses, particularly when steroid excess is absent.
