    Summary

    A new classification for gastric ulcers, type IV, is proposed for juxtacardial ulcers near the esophagogastric junction. These ulcers present unique clinical and pathophysiologic features, including a high incidence of bleeding.

    Background:

    • The Johnson classification for gastric ulcers, established in 1965, is widely used.
    • A high frequency of subcardial or juxtacardial ulcers near the esophagogastric junction has been observed but not previously classified.
    • These 'high' gastric ulcers exhibit distinct clinical and pathophysiologic characteristics.
