Summary

Purpose of the Study:

  • To evaluate the repeatability of brain diffusion-based stiffness prediction (DWIstiff) in healthy volunteers.
  • To compare the repeatability of DWIstiff using two different b-value sets (b200-1500 s/mm2 and b200-1000 s/mm2).

Main Methods:

  • Thirty-one healthy volunteers underwent DWIstiff imaging with two b-value sets, each scanned twice.
  • Regions of interest were delineated by two observers.
  • Repeatability coefficient (RC), coefficient of variation (CV), and Bland-Altman limits of agreement (LoA) were calculated.

Main Results:

  • Twenty-eight participants completed the study.
  • Repeatability varied by brain region, with the brain stem showing the highest RCs (1.17 for DWIstiff, 1500 and 1.58 for DWIstiff, 1000).
  • Whole brain CV was 3.83% for DWIstiff, 1500 and 4.93% for DWIstiff, 1000. Inter- and intraobserver agreement showed acceptable LoA.

Conclusions:

  • DWIstiff is a precise technique for assessing brain stiffness, though some observer dependence exists.
  • Repeatability is higher with the b200-1000 s/mm2 b-value set compared to b200-1500 s/mm2.
  • DWIstiff can reliably detect stiffness changes exceeding 4.93% in healthy individuals.
