    Enteric-coated salicylate overdose can cause delayed absorption, even with treatment. The standard Done nomogram is unreliable for assessing these sustained-release overdoses initially.

    Background:

    • Salicylate poisoning is a common emergency.

    Observation:

    • A patient presented with an enteric-coated salicylate overdose.
    • Initial serum salicylate levels were undetectable (0 mg%) at 1 and 3 hours post-ingestion.
    • Gastric lavage yielded no initial salicylate.
    • Peak serum salicylate levels (35.8 mg%) were observed 24 hours after ingestion, indicating significantly delayed absorption.

    Findings:

    • Absorption of enteric-coated salicylates is significantly delayed.
    • Standard pharmacokinetic assessments may be misleading in the early stages.
    • The Done nomogram is not useful for initial evaluation of sustained-release salicylate overdoses.

    Implications:

    • Clinical management of enteric-coated salicylate overdose requires prolonged monitoring.
    • Delayed absorption necessitates a re-evaluation of standard overdose assessment protocols.
    • Further research is needed to establish effective monitoring strategies for delayed-release salicylate toxicity.
