    Summary

    Salicylates (ASA) combined with phenytoin (DPH) significantly alter thyroid function tests (TFTs). This interaction can lead to further decreases in thyroid hormones, requiring careful interpretation of TFT results in patients on both medications.

    Background:

    • Phenytoin (DPH) is known to affect thyroid function tests (TFTs).
    • Salicylates, such as aspirin (ASA), can also influence thyroid hormone levels.

    Purpose of the Study:

    • To investigate the combined effects of ASA and DPH on TFTs in adult patients.
    • To understand how ASA modifies DPH's impact on thyroid hormone levels.

    Main Methods:

    • Adult patients receiving DPH to achieve steady-state serum levels.
    • Stepwise administration of ASA (325, 650, 975 mg q 4hr) at 2-day intervals.
    • Measurement of TFTs, total serum, and free salivary DPH levels.

    Main Results:

    • Therapeutic DPH levels decreased free thyroxine (FT4), total T4, and total T3 (TT3), while slightly increasing T3 resin uptake (T3RU).
