Background:

Clinical assessment of enthesitis and synovitis in preclinical psoriasis lacks diagnostic accuracy.
Early detection of subclinical joint and tendon inflammation is crucial for timely intervention.

Purpose of the Study:

To evaluate subclinical synovitis and enthesitis in psoriatic patients using ultrasound.
To compare ultrasound findings in psoriatic patients with those in healthy controls.

Main Methods:

A cross-sectional study involving 40 psoriatic patients and 40 age- and sex-matched healthy controls.
Ultrasound examination of 18 joints and 22 entheseal sites on the limbs.
Scoring of ultrasound abnormalities including inflammatory, damage, and total scores.

Main Results:

Psoriatic patients exhibited significantly higher rates of ultrasound-detected enthesitis (92.5%) compared to controls (40%).
Elevated inflammatory, damage, and total scores for enthesitis were observed in psoriatic patients.
Scalp psoriasis was associated with increased ultrasound-detected enthesitis.

Conclusions:

Ultrasound effectively identifies subclinical enthesitis and synovitis in psoriatic patients.
Further prospective research with larger cohorts is recommended to establish ultrasound's role in predicting psoriatic arthritis onset.
