Granulocyte activation during hemodialysis.

    Summary

Different hemodialysis membranes impact white blood cell activation. Polysulfone dialyzers showed minimal increases in granulocyte markers, suggesting they are a safer option for patients undergoing hemodialysis.

    Area of Science:

    • Biomedical Engineering
    • Nephrology
    • Immunology

    Background:

    • Hemodialysis is a life-sustaining treatment for kidney failure.
    • Dialyzer membranes can interact with blood components, potentially causing inflammation.
    • Granulocyte activation markers are indicators of this inflammatory response.

    Purpose of the Study:

    • To investigate the impact of various hemodialysis dialyzer membranes on plasma levels of granulocyte activation markers.
    • To compare the inflammatory potential of different dialyzer materials.

    Main Methods:

    • Patients undergoing maintenance hemodialysis were studied.
    • Plasma levels of lactoferrin, myeloperoxidase, and elastase in complex with alpha 1-proteinase inhibitor (E-alpha 1PI) were measured.
    • Dialyzers made from cellulose hydrate, polymethylmethacrylate, cuprophan, ethylene-vinyl alcohol copolymer, polycarbonate, polyacrylonitrile, or polysulfone were used.

    Related Experiment Videos

    Main Results:

    • Polymethylmethacrylate and cellulose hydrate membranes caused significant increases in E-alpha 1PI and lactoferrin.
    • Polyacrylonitrile membranes elevated lactoferrin but had minimal E-alpha 1PI formation.
    • Polysulfone dialyzers resulted in only modest increases in E-alpha 1PI and lactoferrin.
    • Myeloperoxidase levels remained unchanged across different dialyzers.

    Conclusions:

    • Dialyzer membrane material significantly influences granulocyte activation during hemodialysis.
    • Polysulfone membranes appear to be associated with lower levels of granulocyte activation compared to others.
    • Granulocyte activation during hemodialysis may not always involve anaphylatoxin formation.
