Isolated terminal ileal ulcers (ITIUs) often resolve on their own. Observational follow-up is a viable strategy for patients with these ulcers, as further investigation or therapy did not significantly alter outcomes.
Area of Science:
- Gastroenterology
- Internal Medicine
- Diagnostic Endoscopy
Background:
- Isolated terminal ileal ulcers (ITIUs) are potential indicators of underlying intestinal conditions.
- Current clinical guidelines for managing ITIUs are lacking.
Purpose of the Study:
- To investigate the diagnostic yield of further small intestine examination in patients with ITIUs.
- To evaluate the prognosis and optimal follow-up strategy for patients with undetermined ITIUs.
Main Methods:
- Retrospective screening of patients undergoing colonoscopy between March 2014 and July 2019.
- Inclusion of patients diagnosed with ITIUs, with some undergoing extended small bowel examination.
- Follow-up of undiagnosed patients for at least one year via telephone to gather prognostic data.
Main Results:
- 120 patients (0.3%) were identified with ITIUs.
- Further small intestine examination in 14 patients yielded limited diagnostic benefit.
- No significant difference in prognosis was observed between empiric therapy and observational follow-up groups over a mean of 35.4 months.
- Most patients showed stable or improved clinical and endoscopic outcomes, irrespective of interventions.
Conclusions:
- Nonspecific isolated terminal ileal ulcers generally exhibit a benign prognosis.
- Observational follow-up emerges as a reasonable and effective management strategy for these patients.

Inflammatory Bowel Disease I: Ulcerative Colitis
Inflammatory bowel disease, or IBD, encompasses a group of disorders characterized by chronic inflammation or ulceration of the gastrointestinal tract.
Risk Factors
The exact cause of IBD remains unclear, although it is believed to be due to a mix of genetic, environmental, microbial, and immune factors. Genetic factors are significant in determining susceptibility to IBD, with family history being a critical risk factor. Individuals with a first-degree relative who has IBD are at...
Inflammatory Bowel Disease V: Surgical Management
Here are some common surgical interventions for IBD:
Chronic Bowel Disorders: Introduction
Irritable Bowel Syndrome (IBS) is a common disorder affecting the gastrointestinal tract. The distinctive feature is recurrent abdominal pain associated with altered bowel movements, manifesting as constipation, diarrhea, or fluctuating between both. The...
Inflammatory Bowel Disease II: Crohn's Disease
Inflammatory bowel disease, commonly known as IBD, refers to a collection of disorders that lead to persistent inflammation of the gastrointestinal tract. The two types of IBD are ulcerative colitis, which impacts the colon, and Crohn's disease, which can involve any part of the gastrointestinal segment.
Crohn's disease
Crohn's disease is a chronic, systemic inflammatory bowel disease (IBD) that predominantly affects the gastrointestinal tract. It is marked by...
Peptic Ulcer Disease III: Clinical Manifestations and Diagnostic Studies
Few clinical manifestations differentiate gastric ulcers from duodenal ulcers. Distinctions in the location, timing, and pain relief are crucial for healthcare providers in differentiating between gastric and duodenal ulcers during clinical assessments.
Inflammatory Bowel Disease III: Diagnostic Studies and Management I-Nutritional Therapy
Diagnostic studies
A colonoscopy is the definitive screening test, distinguishing ulcerative colitis from other colon diseases with similar symptoms. During a colonoscopy test, inflamed mucosa with exudate ulcerations can be observed, and biopsies are taken to determine the histologic characteristics of the...
