Background:

Abdominal aortic calcification (AAC) may reflect intrahepatic immunity and influence tumor progression.
The prognostic impact of AAC in colorectal cancer (CRC) patients undergoing resection of liver metastases is not well understood.

Purpose of the Study:

To investigate the association between abdominal aortic calcification (AAC) and prognosis in patients with resected colorectal liver metastases (CRLM).

Main Methods:

A cohort of 99 patients undergoing hepatectomy for CRLM between 2010 and 2019 was analyzed.
Propensity score matching was used to control for baseline patient and tumor characteristics.
Multivariate Cox regression identified risk factors for overall survival and recurrence.

Main Results:

High AAC volume was associated with significantly worse overall survival (OS) and higher remnant liver recurrence rates (RR) post-surgery.
High AAC volume independently predicted poor OS and liver RR, but not lung RR.
Lower expression of tumor necrosis factor-related apoptosis-inducing ligand in liver natural killer (NK) cells was observed in patients with high AAC.

Conclusions:

Elevated AAC volume is strongly correlated with increased liver recurrence after curative CRLM resection.
High AAC may suppress anti-tumor immunity mediated by liver NK cells, leading to poorer prognosis and higher recurrence risk.
