Background:

  • Acute appendicitis (AA) is the most common surgical emergency, but its etiology remains unclear.
  • Emerging evidence suggests an allergic component, involving mast cells and Th2 immune responses, in AA pathophysiology.
  • Mediators like histamine, serotonin, and tryptase are implicated in allergic reactions.

Purpose of the Study:

  • To investigate the role of mast cell mediators in acute appendicitis.
  • To measure histamine, serotonin, and tryptase concentrations in appendicular lavage fluid (ALF) and serum of AA patients and controls.
  • To explore the diagnostic and differential diagnostic potential of these mediators.

Main Methods:

  • Prospective single-centre study involving 22 phlegmonous AA, 24 gangrenous AA patients, and 14 controls.
  • Measurement of histamine and serotonin via ELISA, and tryptase via ImmunoCAP System in ALF and serum.
  • Histological grouping of appendicitis cases.

Main Results:

  • Tryptase levels were significantly higher in both phlegmonous and gangrenous appendicitis patients compared to controls (p < 0.001).
  • ALF tryptase levels were markedly elevated (109-114 times higher) in appendicitis groups versus controls.
  • Serum tryptase showed good discriminative power for AA diagnosis (AUC = 0.825) but weak power for differentiating phlegmonous from gangrenous appendicitis.

Conclusions:

  • Mast cell activation, indicated by elevated tryptase, is involved in the pathogenesis of both phlegmonous and gangrenous appendicitis.
  • Findings support a hypersensitivity type I reaction and Th2 immune response in AA.
  • Histamine and serotonin levels did not differ significantly, possibly due to short half-lives.
