Background:

  • Overweight and obesity in older adults sustain chronic inflammation, enhancing immunosenescence and increasing comorbidity risk.
  • This study investigates the impact of excess body mass on T lymphocyte distribution and the diagnostic utility of anthropometric indices and CD4/CD8 ratio in assessing immune aging.

Purpose of the Study:

  • To determine if excess body mass affects T lymphocyte proportions in older adults.
  • To evaluate the diagnostic usefulness of anthropometric indices and the CD4/CD8 ratio in analyzing immunosenescence.

Main Methods:

  • 103 individuals aged 73.6 ± 3.1 years were grouped by BMI into normal (18.5-24.9 kg/m²), pre-obesity (25.0-29.9 kg/m²), and obesity (≥30.0 kg/m²).
  • Measurements included anthropometry (bioelectrical impedance), biochemical analysis, T and B lymphocyte counts (flow cytometry), and indices like BAI, ABSI, and LAP.
  • Data were analyzed to assess relationships between body mass, T lymphocyte subsets, and immune risk profiles.

Main Results:

  • Obesity group showed the highest CD4+ T cells (59.4%) and lowest CD8+ T cells (31.6%).
  • CD4/CD8 ratio > 2.5 was observed in over 20% of pre-obesity and obesity groups, while < 1 in 64.5% of the normal body mass group.
  • Body Adiposity Index (BAI) and Lipid Accumulation Product (LAP) demonstrated high diagnostic utility (AUC ~0.800-0.900) in distinguishing between body mass groups.

Conclusions:

  • The CD4/CD8 ratio in normal vs. obesity models (OR = 16.1) suggests diagnostic value for clinical prognosis of immune aging related to excess body weight.
  • Combined CD4/CD8 with BAI, LAP, or ABSI provided excellent discrimination (AUC 0.865-1.00) between older adults with obesity and normal body mass.
