Background:

  • Upper Septal Idiopathic Left Ventricular Tachycardia (US-ILVT) is a rare arrhythmia.

Observation:

  • A 71-year-old patient presented with palpitations and NYHA II symptoms.
  • 12-lead ECG revealed US-ILVT, successfully managed initially with Verapamil and beta-blockers.
  • Electrophysiology study identified a re-entrant circuit at the left anterior fascicle (LAF).

Findings:

  • Cardiac MRI showed intramyocardial late gadolinium enhancement (LGE) in the interventricular septum and infero-lateral wall, suggesting prior myocarditis.
  • Ablation of the LAF successfully treated the US-ILVT.
  • An implantable cardioverter defibrillator (ICD) was implanted for secondary prevention.
