Purpose of the Study:

  • To identify predictors of future moderate COPD exacerbations in the general population.
  • To develop a clinical prediction tool for exacerbations in COPD patients.

Main Methods:

  • Analysis of 96,462 individuals from the Copenhagen General Population Study.
  • Identification of 3175 individuals with clinical COPD (FEV1/FVC < 0.70 and FEV1 < 80% predicted).
  • Assessment of various clinical factors including age, sex, FEV1, mMRC, smoking, and comorbidities for 1- and 3-year exacerbation risk.

Main Results:

  • Exacerbation history (HR: 8.53), low FEV1 (HR: 4.82), and higher age (HR: 1.46) significantly predicted future exacerbations in treatment-naïve COPD patients.
  • Male sex and mMRC ≥2 showed borderline significance in predicting exacerbations in patients on maintenance treatment.

Conclusions:

  • Exacerbation history, advanced age, and low FEV1 are significant predictors of future exacerbations in general population COPD.
  • These factors can be utilized to develop a prediction tool for COPD exacerbations.
