Summary

Tarlov cysts, fluid-filled sacs near the spinal cord, can rarely cause symptoms like pelvic pain and premature ejaculation. Surgical treatment for a sacral Tarlov cyst relieved these rare symptoms in one patient.

Observation:

  • A 32-year-old male presented with a rare combination of chronic pelvic pain and acquired premature ejaculation.
  • Clinical evaluation and advanced imaging revealed an intraspinal sacral Tarlov cyst as the likely etiology for his symptoms.

Findings:

  • Surgical intervention, including spinal nerve root decompression, excision of the intraspinal Tarlov cyst, and release of spinal nerve root adhesions, was performed.
  • The patient experienced significant improvement in both pelvic pain and premature ejaculation six months post-surgery.

Implications:

  • This case highlights the potential for intraspinal Tarlov cysts to present with unusual urological symptoms, specifically premature ejaculation.
  • Surgical management can be effective in alleviating symptoms associated with symptomatic Tarlov cysts.
  • Further research into the neuro-urological effects of Tarlov cysts may be warranted.
