Background:

  • Endoscopic retrograde cholangiopancreatography (ERCP) is challenging in patients with surgically altered anatomy, particularly after duodenal switch (DS) procedures.
  • Loop DS, also known as single anastomosis duodeno-ileostomy (SADI) or stomach intestinal pylorus sparing procedure (SIPS), presents unique anatomical challenges for ERCP.
  • Trans-enteric rendezvous ERCP has been rarely reported in DS patients and not previously documented in loop DS anatomy.

Observation:

  • A 50-year-old male with a history of loop DS presented with gallstone pancreatitis.
  • During laparoscopic cholecystectomy, a positive intraoperative cholangiogram indicated a need for biliary intervention.
  • The patient required a trans-enteric rendezvous ERCP to address the biliary obstruction.

Findings:

  • Trans-enteric rendezvous ERCP was successfully performed in a patient with loop DS anatomy.
  • This procedure effectively relieved the biliary obstruction in the described case.
  • This represents the first reported instance of trans-enteric rendezvous ERCP in a loop DS (SADI/SIPS) patient.

Implications:

  • Trans-enteric rendezvous ERCP is a viable and feasible option for managing biliary obstruction in patients with loop DS anatomy.
  • This technique expands the interventional options for managing complex biliary conditions in patients with bariatric surgical alterations.
  • Further studies and case reports are warranted to establish the broader applicability and safety of this approach in similar patient populations.
