  • A 40-year-old male presented with sudden left eye redness, swelling, periorbital ecchymosis, and subconjunctival hemorrhage.
  • Initial topical treatment provided temporary relief, but was followed by progressive blurred vision and axial proptosis.

Findings:

  • Radiological imaging identified an intraconal soft tissue mass involving the left medial rectus muscle.
  • Histopathological examination of the surgically excised mass confirmed it to be a spindle cell hemangioma.

Implications:

  • This case highlights an atypical, acute presentation of orbital spindle cell hemangioma.
  • Prompt surgical intervention (orbital decompression and mass excision) led to complete visual recovery.
  • Clinicians should consider rare orbital tumors in the differential diagnosis of acute-onset visual disturbances and proptosis.
