    Background:

    • Leprosy, caused by Mycobacterium leprae, is a chronic infectious disease primarily affecting the skin and peripheral nerves.
    • Early detection of leprosy is crucial for effective treatment and preventing disability.
    • The initial clinical manifestations of leprosy, particularly in lepromatous forms, are not always well-defined.

    Purpose of the Study:

    • To investigate the initial lesion development following experimental infection with Mycobacterium leprae in an animal model.
    • To determine if localized nodules at the inoculation site are an early indicator of disseminated M. leprae infection.
    • To inform potential strategies for early detection of leprosy in human populations.

    Main Methods:

    • Armadillos were experimentally infected with Mycobacterium leprae via intravenous and intracutaneous routes.
    • Infection doses were standardized at 10(8) M. leprae intravenously and 10(7) M. leprae intracutaneously.
    • Lesion development and dissemination were monitored following inoculation.

    Main Results:

    • A nodule at the inoculation site was the first observed lesion in 14 of 16 intravenously infected armadillos with disseminated disease.
    • In armadillos infected intradermally, a nodule at the inoculation site was the initial lesion in all 4 cases that developed disseminated disease.

    Conclusions:

    • Localized nodule formation at the site of entry may represent an early sign of Mycobacterium leprae infection.
    • The study suggests that in human leprosy, particularly lepromatous leprosy, the initial manifestation could be a localized lesion.
    • Screening for asymptomatic swelling or keloid-like lesions in skin or nasal mucosa may aid in the early detection of leprosy.
