Methods of Documentation III: PIE01:21

Methods of Documentation III: PIE

1.4K
Problem-intervention-evaluation (PIE) is a systematic approach to documentation used in healthcare settings for clinical decision-making and patient care planning. It is a structured approach to organizing patient data based on problems, interventions, and evaluations. Here's a breakdown of its key features and considerations:

Nursing Process for Patient and Caregiver Teaching I: Assessment and Diagnosis

1.6K
The nursing process provides a clinical decision-making framework for patients and families to establish and implement a personalized care plan. Since part of the nurse's duties is to teach patients, the steps of the nursing process are the most effective way to approach instruction. The nursing process and the teaching-learning process are inextricably linked.
It is critical to determine the patient's learning needs during the assessment. Determination of learning needs compounds data...

Methods of Documentation II: POMR

951
The Problem-Oriented Medical Record (POMR) revolutionized medical record-keeping by introducing a systematic approach focusing on the patient's problems rather than merely listing symptoms. Dr. Lawrence Weed's introduction of this method in the 1960s marked a significant advancement in medical documentation. The POMR framework consists of four key components: the database, problem list, plan of care, and progress notes.

Nursing Process for Patient and Caregiver Teaching III: Evaluation and Documentation

1.7K
Evaluation of the teaching process enables the nurse to determine if the patient's learning needs were met and if training was effective. If the expected outcomes are not met, the care plan is revised, and additional education or reinforcement is provided. Nurses can ask questions after the session or obtain feedback to assess the patient's understanding of the topic.
Nurses can use several methods to evaluate patient outcomes. For example, oral questions can assess cognitive learning,...

Assessment of the Gastrointestinal System II: Health Perception Pattern

115
Assessing the gastrointestinal (GI) system is a complex process that begins with collecting subjective data. This data, collected through patient interviews, provides crucial insights into the patient's health history, perception patterns, and lifestyle habits, all contributing significantly to GI health.
Health Perception Patterns
Health perception patterns offer valuable insights into a patient's lifestyle habits and how they may impact their GI health. These patterns include:

Data Collection III

2.8K
The physical assessment examines the patient for objective data that defines the patient's condition, and aids in formulating the nursing care plan. The purpose of physical assessment is a health status appraisal, which includes identifying health problems, and establishing a database for nursing intervention.
The principles to begin the physical assessment include conducting a comprehensive or problem-related history in a quiet, well-lit room, emphasizing privacy and comfort for the...

Summary

A needs assessment for pythiosis continuing professional development (CPD) identified knowledge gaps. The CPD program should focus on symptomatology, laboratory investigation, and epidemiology for early pythiosis detection.

Area of Science:

  • Medical Education
  • Infectious Diseases
  • Public Health

Background:

  • Pythiosis is a rare, highly fatal disease, predominantly reported in Thailand and India.
  • Early diagnosis and surgical intervention are crucial for improving patient outcomes.
  • Continuing professional development (CPD) is vital for timely pythiosis recognition, yet a needs assessment for a CPD program is lacking.

Purpose of the Study:

  • To conduct a needs assessment for developing a pythiosis CPD program.

Main Methods:

  • A survey study was administered to 267 residents at King Chulalongkorn Memorial Hospital.
  • The survey included knowledge assessment, demographics, and attitudes, distributed electronically and on paper.
  • Data were analyzed using descriptive and inferential statistics.

Main Results:

  • The mean knowledge assessment score was 41.67%, with the lowest scores in laboratory investigation (15.00%), epidemiology (29.17%), and symptomatology (30.83%).
  • Participants preferred an online CPD program with synchronous and asynchronous sessions, 60-90 minutes in length.
  • High-scoring domains included risk factors (67.22%), microbiologic characteristics (50.83%), and radiographic interpretation (50.56%).

Conclusions:

  • The pythiosis CPD program must prioritize education on symptomatology, laboratory investigation, and epidemiology for early detection.
  • Implementing a virtual blended format for the CPD program is recommended.
  • Addressing these educational needs is critical to reduce mortality from pythiosis.
