Background:

Littre's hernia is an exceptionally rare condition characterized by the presence of Meckel's diverticulum within the hernia sac.
Umbilical hernias are common, but Littre's hernia is an infrequent presentation.

Observation:

A 63-year-old male presented with symptoms of an incarcerated umbilical hernia.
Intraoperative findings revealed an umbilical Littre's hernia, with Meckel's diverticulum as the herniated content.

Findings:

The patient underwent successful open Meckel's diverticulectomy and mesh repair.
Preoperative diagnosis is challenging due to the rarity and nonspecific clinical/radiological findings of Littre's hernia.

Implications:

Computed tomography (CT) and ultrasound play vital roles in distinguishing between incarcerated omentum and bowel.
Accurate preoperative imaging is essential for determining the urgency of surgical intervention in suspected Littre's hernia cases.
