    Summary

    Dacryoscintigraphy is a more sensitive imaging method for diagnosing epiphora (watery eye) than conventional radiography. Normal results from dacryoscintigraphy indicate a patent tear duct, making further imaging unnecessary.

    Background:

    • Epiphora, or watery eye, is a common condition requiring accurate diagnostic imaging.
    • Conventional contrast radiography (macrodacryocystography) has been used to evaluate tear duct obstruction.
    • The diagnostic accuracy of dacryoscintigraphy compared to macrodacryocystography for epiphora has not been definitively established.

    Purpose of the Study:

    • To compare the sensitivity of dacryoscintigraphy and macrodacryocystography in diagnosing tear duct obstruction in patients with epiphora.
    • To determine the optimal imaging sequence for investigating epiphora.

    Main Methods:

    • Fifty-three patients with epiphora underwent bilateral dacryoscintigraphy.
    • Following scintigraphy, 66 tear ducts were evaluated using bilateral or unilateral macrodacryocystography.
    • Sensitivity for obstruction was calculated for both imaging modalities.

    Main Results:

    • Dacryoscintigraphy identified obstruction in 77% of systems (51/66), while macrodacryocystography identified obstruction in 51% (33/66).
    • Normal dacryoscintigraphy results consistently correlated with patent tear ducts on macrodacryocystography.
    • Digital subtraction imaging for lacrimal passages was discussed as a potential combined imaging technique.

    Conclusions:

    • Dacryoscintigraphy is a more sensitive initial investigation for epiphora than macrodacryocystography.
    • If dacryoscintigraphy is normal, further contrast radiography is not required, streamlining the diagnostic process.
    • Digital subtraction imaging may offer a comprehensive approach to evaluating the lacrimal system.
