Summary

Complicated trabeculectomy surgery can be converted to ab externo cyclodialysis. This technique successfully reduced intraocular pressure in a congenital glaucoma patient with a scleral flap complication.

Background:

Trabeculectomy is a common glaucoma surgery.
Complications during scleral flap creation can necessitate procedure abortion.
Congenital glaucoma presents unique surgical challenges.

Observation:

A patient with congenital glaucoma experienced a scleral wall perforation with exposed choroid during trabeculectomy flap dissection.
A Barraquer cyclodialysis spatula was used through the perforation to perform ab externo cyclodialysis.
The choroid was separated from the sclera up to the scleral spur over 30 degrees into the anterior chamber.

Findings:

The complicated trabeculectomy was successfully converted to ab externo cyclodialysis.
The patient's intraocular pressure was reduced to 16 mmHg two months post-surgery.
No hypotensive topical medications were required post-operatively.

Implications:

This technique offers an alternative management for perioperative complications during trabeculectomy.
Ab externo cyclodialysis can be a viable salvage procedure.
Successful IOP reduction was achieved with a minimally invasive conversion technique.
