Valgus malalignment causes increased forces on a medial collateral ligament reconstruction under dynamic valgus

- For chronic medial instabilities with ≥5° valgus deformity, concurrent realigning osteotomy with MCLR is recommended to enhance graft protection and reduce failure rates.
