Nephrons

2.4K
The kidneys are intricate organs with millions of working units known as nephrons. Each nephron features two major structures: the renal corpuscle, which facilitates blood plasma filtration, and the renal tubule, which handles the glomerular filtrate. Blood supply is directly linked to the nephrons. The renal corpuscle consists of the glomerulus, a capillary network, and the Bowman's capsule, a double-walled epithelial structure that encases the glomerulus. The filtering of blood plasma...

Endoscopic Procedures V: ERCP

Endoscopic Retrograde Cholangiopancreatography (ERCP) is a diagnostic procedure that combines endoscopy and fluoroscopy to diagnose and treat conditions related to the bile ducts, pancreatic ducts, and gallbladder. This procedure is beneficial for identifying and addressing blockages, gallstones, strictures, and tumors within the biliary or pancreatic systems. ERCP is both diagnostic and therapeutic, offering the ability to visualize and treat identified problems in one session.

Summary

Mini-percutaneous nephrolithotomy (PCNL) significantly improves kidney function in patients with chronic kidney disease (CKD) and kidney stones. This minimally invasive procedure offers high stone clearance and comparable safety to standard PCNL.

Area of Science:

  • Urology
  • Nephrology
  • Minimally Invasive Surgery

Background:

  • Urinary stone disease affects 3.2% of patients with end-stage renal disease (ESRD), potentially causing renal damage.
  • Percutaneous nephrolithotomy (PCNL) is the gold standard for complex kidney stones, with advancements in technology leading to smaller instruments.
  • Chronic kidney disease (CKD) complicates stone management, necessitating safe and effective treatment options.

Purpose of the Study:

  • To evaluate the safety and feasibility of mini-PCNL in patients with CKD.
  • To assess the impact of mini-PCNL on renal function, specifically estimated glomerular filtration rate (eGFR).
  • To determine the effect of mini-PCNL on CKD stage and postoperative outcomes.

Main Methods:

  • A prospective study included 46 adult patients with nephrolithiasis and CKD stage 3 or higher.
  • Mini-PCNL was performed, and eGFR was measured using the MDRD equation at baseline and 6-month follow-up.
  • Secondary outcomes included complication rates, stone clearance, and hospital stay duration.

Main Results:

  • Mini-PCNL demonstrated a significant improvement in eGFR at 6 months (mean difference 14.25 ml/min/1.73 m²; p<0.01).
  • High stone-free rates of 89.5% were achieved.
  • Complication rates were comparable to those of standard PCNL.

Conclusions:

  • Mini-PCNL is a safe and effective treatment for kidney stones in CKD patients.
  • The procedure leads to significant renal function improvement and high stone clearance.
  • Mini-PCNL represents a valuable therapeutic option for managing nephrolithiasis in the context of CKD.
