Summary

Arthroscopic hip surgery significantly improves depression in patients with preoperative symptoms. Those with preoperative clinical depression (CD) achieved similar functional outcomes to non-depressed patients post-surgery.

Background:

  • Femoroacetabular impingement syndrome (FAIS) can impact quality of life.
  • Preoperative depressive symptoms may affect surgical outcomes.
  • Assessing depression is crucial for comprehensive patient care in orthopedic procedures.

Purpose of the Study:

  • To evaluate clinical depression scores and functional outcomes after arthroscopic treatment for FAIS.
  • To compare outcomes in patients with and without preoperative depressive symptoms, as defined by the Patient-Reported Outcomes Measurement Information System for Depression (PROMIS-D).

Main Methods:

  • 100 FAIS patients completed PROMIS-D and other PROs preoperatively and postoperatively.
  • Patients were grouped into clinically depressed (CD, PROMIS-D score ≥55) and nonclinically depressed (NCD) based on preoperative scores.
  • Outcomes were assessed at 6 months and ≥1 year postoperatively.

Main Results:

  • 21% of patients had preoperative CD; no demographic or radiographic differences were noted between CD and NCD groups.
  • The CD group showed significant improvement in PROMIS-D scores at 1 year (-9.1 vs -0.8, P=.001).
  • No significant difference in achieving patient acceptable symptom state was found between CD and NCD groups.

Conclusions:

  • Arthroscopic hip surgery leads to significant improvement in depressive symptoms for FAIS patients with preoperative CD.
  • Preoperative depression does not negatively impact the achievement of minimum clinically important difference or patient acceptable symptom state post-arthroscopy.
