Summary

Resective epilepsy surgery can improve seizure control. For patients with language area involvement, careful preoperative planning is crucial for balancing seizure freedom and preserving function.

Background:

  • Intractable epilepsy often requires surgical intervention for seizure control.
  • Noninvasive methods like MRI and EEG localize seizure foci, but intracranial EEG (iEEG) may be necessary.
  • Subdural grid electrodes (SDG) and stereotactic EEG are primary iEEG techniques.

Observation:

  • A case of temporal lobe epilepsy with subtle MRI lesions in the language-dominant superior temporal sulcus is presented.
  • SDG and functional mapping revealed seizure onset zone overlap with critical language areas.
  • Resection involved the middle and inferior temporal gyri, impacting the language area.

Findings:

  • The patient achieved Engel IIB seizure outcomes post-resection.
  • Surgical intervention in eloquent areas carries risks to functional preservation.
