Summary

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection can trigger autoimmune responses. This case highlights polymyalgia rheumatica as a rare post-COVID-19 rheumatic manifestation, successfully treated with prednisone.

Background:

  • Growing evidence links SARS-CoV-2 infection to immune dysregulation and autoimmune phenomena.
  • Post-viral syndromes can manifest with diverse rheumatological symptoms.

Observation:

  • A 75-year-old woman presented with acute neck and shoulder pain, stiffness, and fatigue post-COVID-19.
  • Clinical presentation and ultrasound revealed bilateral subacromial-subdeltoid bursitis, initially suggestive of polymyalgia rheumatica (PMR).

Findings:

  • Despite negative C-reactive protein, symptoms aligned with PMR criteria.
  • Treatment with prednisone led to complete recovery within six months.

Implications:

  • COVID-19 infection may precipitate autoimmune rheumatic conditions like PMR.
  • PMR should be considered in the differential diagnosis of rheumatological symptoms following SARS-CoV-2 infection.
