Chronic Pancreatitis I: Introduction

87
The pancreas, an elongated and flat gland situated behind the stomach, serves a vital function in digesting food and managing blood sugar levels.
Pancreatitis is the inflammation of the pancreas, which occurs when the immune system becomes active and causes swelling, pain, and disruptions in organ function. Pancreatitis can manifest as either an acute or chronic condition.
Acute pancreatitis arises suddenly and lasts for a brief duration, while chronic pancreatitis is a long-term affliction...

    Summary

Laparoscopic distal pancreatectomy offers benefits through a minimally invasive approach, though open surgery may be preferred for malignant conditions. This study compared outcomes of both methods over a decade.

    Area of Science:

    • Surgical Oncology
    • Gastrointestinal Surgery
    • Minimally Invasive Surgery

    Background:

    • Distal pancreatectomy is a standard procedure for pancreatic body/tail lesions.
    • Surgical resection is the only curative option for pancreatic adenocarcinoma.

    Purpose of the Study:

    • To assess and compare perioperative and postoperative outcomes of laparoscopic versus open distal pancreatectomy.
    • To evaluate the safety and efficacy of minimally invasive techniques in pancreatic surgery.

    Main Methods:

    • Retrospective analysis of 112 distal pancreatectomies performed between 2011-2021.
    • Comparison of outcomes between laparoscopic (67 patients) and open laparotomy (45 patients) approaches.
    • Analysis included operating time, blood loss, hospitalization, and complication rates (e.g., fistulas).

    Main Results:

    • Laparoscopic procedures were more frequent (59.8%) but had higher rates of biochemical leak and Grade B fistula.
    • Open laparotomy was associated with Grade C fistulas.
    • Laparoscopic surgery showed lower blood loss and shorter hospitalization, despite slightly longer operating times.
    • Reoperation rates were similar between groups.

    Conclusions:

    • Laparoscopic distal pancreatectomy is increasingly preferred due to its minimally invasive benefits.
    • A more cautious approach is warranted for malignant disease to ensure radicality.
    • Both techniques have distinct advantages and complication profiles that guide surgical decisions.
