    Summary

    Median palatine cysts are rare, nonodontogenic lesions of the hard palate. These cysts are typically asymptomatic and treated by simple enucleation without recurrence.

    Background:

    • Median palatine cysts are uncommon, nonodontogenic lesions originating from the hard palate.
    • These cysts typically present as asymptomatic, fluctuant swellings, potentially affecting the nasal cavity floor.

    Observation:

    • Grossly, these cysts appear symmetrical along the hard palate's midline, located posterior to the palatine papilla.
    • Radiographically, they present as ovoid or circular lesions, distinct from incisive canal or nonvital teeth.
    • Histologically, the cyst wall comprises fibrous collagenous tissue with chronic inflammatory cells, lined by squamous or respiratory epithelium.

    Findings:

    • Median palatine cysts are differentiated from nasopalatine and other anterior maxillary cysts by specific diagnostic criteria.
    • Absence of involvement with the palatine papilla or incisive canal is a key characteristic.
    • Histological examination excludes nerve trunks, large vascular spaces, cartilage, or salivary gland tissue in the cyst wall.

    Implications:

    • Simple enucleation is an effective treatment, with no reported recurrence.
    • Accurate differentiation from other maxillary cysts is crucial for appropriate management.
    • Understanding the distinct histological and diagnostic features aids in precise diagnosis and treatment planning.
