Background:

A significant percentage of SARS-CoV-2 infections result in post-viral illness, commonly known as Long COVID.
Long COVID can severely impact patients' ability to work, engage in daily activities, and reduce overall quality of life.

Purpose of the Study:

To describe the development and evolution of the Outpatient Post-COVID Condition Rehabilitation Program at Unity Health Toronto.
To outline a multidisciplinary, patient-centered approach to managing Long COVID symptoms and maximizing patient function.

Main Methods:

The program utilized group education and a virtual platform to facilitate peer learning and experience sharing among patients across Ontario.
A multidisciplinary team adapted program content based on emerging research on Long COVID and patient needs over two years.

Main Results:

The program aimed to teach patients symptom management strategies to enhance independence and function.
Continuous adaptation ensured the program remained responsive to the evolving understanding of Long COVID and patient requirements.

Conclusions:

The Outpatient Post-COVID Condition Rehabilitation Program provided a structured, supportive environment for Long COVID recovery.
The program's evolution demonstrates a commitment to evidence-based, patient-centered care for post-viral conditions.
