Area of Science:

  • Oncology
  • Cancer Research
  • Genitourinary Cancers

Background:

  • Despite significant progress in germ cell tumor (GCT) treatment, critical challenges impede optimal patient outcomes.
  • Certain patient subgroups, including those with International Germ Cell Cancer Collaborative Group poor-risk disease or platinum-refractory GCTs, face a poor prognosis.

Approach:

  • This review synthesizes current knowledge on persistent challenges in GCT management.
  • It emphasizes the need for novel therapeutic strategies and diagnostic biomarkers.
  • Highlights the importance of a multidisciplinary approach for complex cases, such as those with brain metastases.

Key Points:

  • Investigating novel therapeutic interventions is essential for patients with poor-risk GCT.
  • A multidisciplinary approach is vital for managing GCT patients with brain metastases.

  • Development of novel biomarkers for minimal residual disease or teratoma detection is needed.
  • Conventional tumor markers (AFP, hCG) remain standard, but improved detection methods are required.
  • Conclusions:

    • Effective management of germ cell tumors necessitates a collaborative, multidisciplinary approach.
    • Patients with GCT should ideally receive care at tertiary centers with specialized expertise.
    • Continued research into novel treatments and biomarkers is critical for improving GCT patient survival and care.
