Background:

  • The link between antibiotic prophylaxis and infective endocarditis (IE) post-dental procedures remains debated.
  • Guidelines in 2007 restricted antibiotic prophylaxis indications.

Purpose of the Study:

  • To systematically review and analyze evidence on antibiotic prophylaxis and IE following invasive dental procedures.
  • To assess the impact of prophylaxis on IE incidence in different risk groups.

Main Methods:

  • A comprehensive search of multiple databases (PubMed, Scopus, etc.) was conducted up to May 2023.
  • Included studies were case-control, cohort, self-controlled case series, and time-trend analyses.
  • Meta-analysis was used to pool relative risk (RR) for IE in patients receiving prophylaxis versus those not.

Main Results:

  • Eight high-quality substudies (case-control/cohort/self-controlled case series) showed mixed results, with 5 supporting a protective role for prophylaxis.
  • Meta-analysis of these studies indicated a significantly lower IE risk in high-risk individuals receiving antibiotic prophylaxis (RR, 0.41).
  • Time-trend studies yielded inconsistent findings, with some showing increased IE incidence and others no significant change or a decrease.

Conclusions:

  • Antibiotic prophylaxis is associated with reduced IE risk in high-risk individuals undergoing invasive dental procedures.
  • No proven association was found for low/unknown risk individuals, supporting current guidelines.
  • Insufficient data exists to confirm benefits for moderate-risk individuals.
