    Impact of HLA mismatch direction on outcomes after umbilical cord blood transplantation for hematological malignant disorders: a retrospective Eurocord-EBMT analysis.

    Background:

    • Dural sinus thrombosis (DST) is a rare but serious complication.
    • This study investigates DST in patients with hematological malignancies, specifically acute lymphoblastic leukemia (ALL) and non-Hodgkin malignant lymphoma (NHML).

    Observation:

    • Eight patients (4 males, 4 females, aged 16-43) developed DST.
    • Five patients had ALL (FAB-2 subtype), and three had NHML.
    • DST occurred during induction therapy or maintenance phase.
    • Neurological symptoms included headache, seizures, and hemiparesis.
    • Contraceptive drug use was noted in three female patients.

    Findings:

    • CT scans revealed edema or hemorrhagic infarction in 4 cases.
    • Angiography confirmed superior sagittal sinus occlusion in 7 cases and transverse sinus occlusion in 1 case.
    • Six patients received anticoagulant therapy.
    • Three patients (2 with ALL, 1 with NHML) died due to tentorial herniation or infection.

    Implications:

    • DST is a significant neurological complication in leukemia patients.
    • Early recognition and management of DST are crucial.
    • Further research is needed to optimize treatment strategies and improve outcomes for these patients.
