    Background:

    • Accurate measurement of incisor angulation is crucial for orthodontic diagnosis and treatment planning.
    • Conventional methods often rely on two-dimensional (2D) radiographic techniques, which may have limitations in capturing true three-dimensional (3D) spatial relationships.

    Purpose of the Study:

    • To compare the accuracy and reliability of 2D radiographic measurements versus 3D dental cast analysis for determining incisor angulations.
    • To evaluate differences in measuring inter-incisor angles and incisor angulation relative to the occlusal plane using both techniques.

    Main Methods:

    • Forty patients' incisor angulations were assessed using both lateral cephalometric radiographs and direct dental cast measurements via a computer-linked Reflex Metrograph.
    • Both 2D radiographic and 2D/3D dental cast methods were employed to measure inter-incisor angles and angulations to the occlusal plane.

    Main Results:

    • Significant differences (p < 0.01) were found between 2D radiographic and 3D dental cast measurements for inter-incisor angulation.
    • Significant differences (p < 0.01) were also observed between 2D radiographic and 2D dental cast measurements for upper incisor angulation to the occlusal plane.
    • Discrepancies are attributed to measuring incisor crowns on casts versus incisor tips/apices on radiographs.

    Conclusions:

    • Dental cast measurements provide a more realistic and reliable assessment of incisal angles compared to conventional radiographic techniques.
    • The study suggests that 3D dental cast analysis may be superior for precise orthodontic evaluations of incisor positioning.
