Existing health technology assessment frameworks need adaptation for digital health technologies (dHTs). This review identified 102 frameworks and proposed a new structure with 12 domains, 38 dimensions, and 11 subdimensions for comprehensive dHT assessment.
Area of Science:
- Health Technology Assessment
- Digital Health Technologies
- Health Systems Research
Background:
- Digital health technologies (dHTs) present opportunities and challenges for healthcare systems globally.
- Assessing the impact and risks of dHTs requires robust evaluation methods.
- Traditional health technology assessment (HTA) frameworks may need adaptation for dHTs.
Purpose of the Study:
- To identify methodological frameworks for digital health technology assessment (dHTA) worldwide.
- To determine the domains considered in existing dHTA frameworks.
- To propose a novel dHTA methodological framework based on literature analysis.
Main Methods:
- A scoping review (ScR) following PRISMA-ScR guidelines.
- Searched 7 databases for peer-reviewed and gray literature (2011-2021).
- Thematic analysis of 26 included studies to identify and organize assessment domains.
Main Results:
- Identified 102 distinct methodological frameworks for dHTA, showing significant heterogeneity.
- Thematic analysis synthesized heterogeneous frameworks into a hierarchical structure.
- A proposed framework includes 12 domains, 38 dimensions, and 11 subdimensions for dHTA.
Conclusions:
- Existing dHTA frameworks require adaptation or new development for comprehensive assessment.
- The identified domains, dimensions, and subdimensions provide a basis for improved dHTA.
- This research offers a structured approach to evaluating the value and risks of dHTs.
