    Summary

    Amsacrine showed limited efficacy in treating relapsed acute leukemia, with only one partial remission observed. The drug caused significant myelosuppression and other toxic effects in heavily pretreated patients.

    Background:

    • Relapsed acute leukemia presents a significant challenge in treatment.
    • Patients with heavily pretreated, relapsed acute leukemia have limited therapeutic options.

    Purpose of the Study:

    • To evaluate the efficacy and safety of amsacrine as a single agent in patients with heavily pretreated, relapsed acute leukemia.

    Main Methods:

    • Amsacrine was administered at a dose of 120 mg/m2/day for 5 days to 32 patients.
    • Thirty-one patients were evaluable for response after receiving a total of 41 courses of therapy.

    Main Results:

    • No complete remissions were observed.
    • One partial remission (3 months) was achieved in an adult patient with acute lymphoblastic leukemia.
    • Common toxicities included myelosuppression (100%), hyperbilirubinemia (41%), and nausea/vomiting (41%).

    Conclusions:

    • Amsacrine monotherapy is not effective for heavily pretreated, relapsed acute leukemia in adults and children.
    • The observed toxicities further limit its utility in this patient population.
